1️⃣ Apply for SSI (Supplemental Security Income)

Start the process around age 17-18. SSI can help provide financial support once your teen becomes an adult.

2️⃣ Set Up a Special Needs Trust or ABLE Account

Protect future savings and benefits eligibility by opening the right financial tools early.

3️⃣ Apply for Medi-Cal or Medicaid (Even if You Have Private Insurance)

Medicaid can cover services private insurance doesn’t including job coaching, therapies, and housing supports.

4️⃣ Request Transition Services in the IEP

Starting at age 16 schools must include transition planning: skills for jobs, college, daily living.

5️⃣ Practice Daily Living Skills

Build routines around cooking, money handling, taking public transit, scheduling appointments, and self-advocacy.

6️⃣ Explore Vocational Rehab or Job Coaching Programs

Look into state-funded programs that provide training, internships, and employment support for individuals with disabilities.

7️⃣ Try Out Job Skills at Home

Work on trying new skills and jobs around the house. Great to practice and understand interests.

8️⃣ Understand Housing Options

Start learning about different setups: living at home, supported living, group homes, or independent apartments with supports.

9️⃣ Secure Guardianship or Supported Decision-Making

Decide if your teen will need legal guardianship, conservatorship, or a supported decision-making agreement after 18.

🔟 Build a Support Circle

Identify trusted family, friends, mentors, or professional advocates who will be part of your teen’s long-term team.

And try to enjoy this process together!
